FOR SUSTAINING PAYMENT PLANS: By enrolling as a Sustaining Member I give my authorization to KCBX, Inc. to contribute monthly from the account I've indicated. This sustaining donation shall remain in effect until I notify KCBX that I wish to change or end it, and KCBX has had reasonable time to act on my wishes. I understand that my contributions will occur monthly. A record of each contribution will appear on my monthly credit/debit card statement as KCBX INC and will serve as my receipt.
